  LifeCare Plus+ Care Provider Membership Information

LifeCare Plus+ Care Provider Referral Service
The LifeCare Plus+ Network is a secure, HIPAA compliant Care Provider Referral Service that places your facility in our national database of Care Providers. From anywhere in the country 24 hours a day, seven days a week, when a patient needs to be placed into a facility matching your scope of health care services and matching your city/county and state location, your facility will receive a patient referral.  

Facility Information Page  
Your facility's location and contact information, scope of health care services provided, amenities, admissions policy, and up to six photographs of your facility are listed on your facility's information page. This page is a great resource of information for hospital patient care managers, patients, and family members. Care Provider members can change and update their facility's information page whenever the need arises. Also, facility's may add a link to an already existing facility web site if they wish to add more information than what is listed on the facility's LifeCare Plus+ information page.

Plus+Mail, The LifeCare Plus+ Network Mail System
Facility Administrators and patient care managers access the LifeCare Plus+ Network Mail System through Plus+Mail. Plus+Mail is a secured, encrypted, and HIPAA compliant messaging system that members use to communicate via email. Patient referrals, messages between facility's and hospital's, and any other email communication that takes place over the mail system is done through the Plus+Mail system. 

LifeCare Plus+ Job Search Database

The LifeCare Plus+ Job Search allows facility Administrators to post job openings at their facility to our nation wide Job Search database.
This feature provides administrators a means to connect to  professionals and paraprofessionals who are seeking work in the long-term care industry.  Facility administrators simply submit their job openings to the Job Search database. Job seekers can search for job openings by City, State, job title, and/or employer name. Job seekers can then respond directly to a job posting by entering information on an on-line job interest form or by communicating directly with a designated contact person at the Administrator's  facility. The LifeCare Plus+ Job Search database is searchable by  everyone. Only members may post job openings.

Nation Wide Hospital Directory Access
Easily find Hospitals by City, County, and/or State.  

Nation Wide Care Provider Directory Access
Easily find other Care Providers by City, County, and/or State.
This service is especially useful for transferring patients from one facility to another facility in a different geographical location.

Facility Enhancement Packages
Enhancement packages are not included in the basic Care Provider Membership Service and require additional fees for each package. 

  • Receive Patient Referrals By Fax Instead Of Email.
    If your facility isn't connected to the internet, we can send your facility's patient referrals directly to your fax machine.

  • Receive Monthly, Quarterly, and Annual Reports
    These reports provide administrators and patient care managers with specific patient placement information in their county and state in the areas of the type of care patients required, the special needs of the patients, and the total number of patients placed into facilities.  

  • Create An Internet Presence For Your Facility.
    If your facility does not already have a web site or if your current web site needs to be updated, let our web professionals create and market your facility's presence on the internet.
